Or you can fill out the original form.
If you know you do not qualify or do not wish to participate there is a box to check on the online application. Or you can just write it on the paper form. You just need to let us know so we do not keep asking you to fill one out.
The new forms will not let you complete the application unless you have all the information, like the CASE #, this information is required for it to be a complete application. Your application will not be verified for free or reduced meals unless it is a completed application.
